Job summary

Sweetwater Care is seeking an attentive LVN/LPN to provide nursing care within scope of practice in accordance with standards, physician orders, and local guidelines. The role emphasizes accurate charting, incident reporting, and collaborative rounds with physicians.

The ideal candidate holds an active LVN/LPN license in California, has LTC experience, and is familiar with PointClickCare. BLS/CPR certification is required, with management experience considered a plus.

Qualifications

• Licensed LVN/LPN in state of practice.
• Current BLS/CPR certification.
• Long Term Care experience.
• Experience with Point Click Care charting.
• Nursing management experience preferred.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents change in patient conditions and notable health changes.
• Uses Quality Life Manual to identify and monitor patients.
• Notifies departments of patient room changes, LOA, admits or discharges.
• Makes rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
