Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care in a long-term care setting in California. The role requires adherence to standards, physician orders, and center policies, with attention to state and local guidelines.

Ideal candidates will have long-term care experience, PCC charting familiarity, and an active LVN/LPN license in the state. BLS/CPR certification is expected, or willing to obtain it. Nurse leadership experience is a plus.

Qualifications

• Long Term Care experience required.
• PointClickCare charting experience preferred.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire.
• Nursing management experience preferred but not required.

Responsibilities

• Follow established charting procedures and incident reporting.
• Follow procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in patient conditions (nutrition, infections, skin issues, weight loss, etc.).
• Use the Quality Life Manual to identify, place, and monitor patients in programs.
• Document and notify departments about room changes, LOA, new admits or discharges.
• Round with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
