Job summary

Sweetwater Care is seeking a qualified licensed practical nurse (LVN/LPN) to provide nursing care within scope of practice in a long-term care setting. The role emphasizes accurate charting, compliance with state guidelines, and collaboration with physicians.

The ideal candidate holds an active LVN/LPN license in the practicing state and a current BLS/CPR certification. Experience with PointClickCare and prior nursing management exposure is preferred but not required.

Qualifications

• Experience in long-term care settings.
• Proficient with PointClickCare charting.
• Currently licensed LVN/LPN in the practicing state.
• BLS/CPR certification or willingness to obtain.

Responsibilities

• Follow established charting procedures and incident reporting.
• Follow company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in patient conditions (nutrition, infections, skin issues, weight loss, etc.).
• Utilize the Quality Life Manual for patient identification, placement, and monitoring in programs.
• Document and notify relevant departments of room changes, LOA, new admits, or discharges.
• Mop rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES
