Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care within scope of practice in a long-term care setting in California. You will perform charting, monitor patient status, and collaborate with physicians during rounds.

The role requires an active LVN/LPN license in California and a current BLS/CPR certification, with preference for candidates who have managed nursing teams. Equal opportunity employer.

Qualifications

• Long Term Care experience required.
• PointClickCare charting experience required.
• Active LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire.
• Nursing management experience is preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions (nutrition, infections, skin issues, weight, etc.).
• Uses Quality Life Manual to identify, place, and monitor patients in programs.
• Documents room changes, LOA, admits, or discharges and notifies departments.
• Rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
