Job summary

Corewell Health seeks a Social Worker to collaborate with an interdisciplinary team, assisting patients and families with complex psychosocial needs. You will coordinate discharge planning, alternate care sites, and community resources, documenting assessments in the EMR and providing consults to the care team.

Requirements include a Master's in Social Work, 2+ years of related experience, and Michigan MSW eligibility. This full-time role is based in Zeeland, MI with standard daytime hours.

Qualifications

• Master's degree in Social Work required; licensure or eligibility preferred.
• Minimum 2 years of relevant experience in social work.
• Healthcare-related experience post Master's Degree is preferred.
• Care management experience is desirable.
• Experience in individual/family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• MSW license or eligibility in Michigan is preferred.
• Upon hire required and a valid Michigan driver's license.
• Healthier Communities involvement is an asset.

Responsibilities

• Collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs.
• Coordinate alternate sites of care, discharge planning, and community referrals.
• Conduct psychosocial assessments and document plans in the EMR with accuracy.
• Provide short-term therapeutic support and resource consultation to the care team.
• Stay current on federal/state regulations related to the role and participate in committees or projects as assigned.
