Qualifications

• Master's degree in Social Work required.
• 2 years of healthcare-related experience preferred.
• Care management experience preferred.
• Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.

Responsibilities

• Collaborate with an interdisciplinary team to support patients and families with psychosocial needs.
• Conduct psychosocial assessments and develop care plans.
• Coordinate with alternate sites of care and community resources; assist with discharge planning.
• Provide short-term therapeutic support and bereavement or counseling resources as appropriate.
