Job summary

Corewell Health is seeking a licensed social worker to join an interdisciplinary team addressing complex psychosocial needs. You will assess patients and families, coordinate post-discharge care, and provide crisis intervention and bereavement support as needed.

Responsibilities include documenting plans in EMR, liaising with community resources, and staying current with regulations. Strong communication and advocacy skills are essential for success in this role.

Qualifications

• Master's degree in Social Work required.
• 2 years of healthcare-related experience post-master's degree preferred.
• Care management experience preferred.
• Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• Licensed MSW in Michigan upon hire required.
• Licensed Driver's License in Michigan upon hire required.

Responsibilities

• Collaborate with an interdisciplinary team to assess psychosocial needs.
• Coordinate with alternate sites of care and community agencies.
• Document assessments and plans in electronic medical records.
• Provide consultation and resources to the healthcare team.
• Maintain knowledge of community resources.
• Participate in committees or projects as assigned.
• Stay updated on federal and state regulations relevant to the role.
• Provide short-term therapeutic support as needed.
