Job summary

UC Health in Cincinnati is seeking a Pre-Transplant Coordinator to drive evaluation scheduling, chart preparation, and interdisciplinary collaboration across transplant teams. You will coordinate with referring providers, nursing colleagues, and UNOS processes to ensure accurate patient data and timely listings.

Requires an Ohio RN license with an associate degree; three to five years nursing experience, with transplant or critical care preferred. ACLS preferred.

Qualifications

• Active Ohio RN license required.
• Associate degree required; bachelor's degree preferred.
• 3–5 years of clinical nursing experience required; transplant/nephrology/hepatology/ICU or complex med-surg experience preferred.

Responsibilities

• Initiate, coordinate, and track pre-transplant evaluations and required testing.
• Prepare evaluation charts, review histories, and synthesize findings for committee presentation.
• Provide education and obtain informed consent for evaluation and listing.
• Collaborate with referring providers, cardiology, and multidisciplinary teams.
• Ensure accuracy and compliance of UNOS listing data, closure letters, and notifications.
• Manage evaluation communication channels (phones, faxes, imaging transfers).
• Support regulatory adherence and process improvement in evaluation workflows.
