Job summary

UC San Diego Health is seeking an Inpatient Abdominal Transplant Coordinator RN to manage care coordination for liver and kidney transplant patients throughout the inpatient continuum. You will educate patients and families, ensure regulatory compliance, and promote high-quality transplant care.

The role requires current acute care RN experience in transplant settings, strong critical thinking, and excellent communication with multidisciplinary teams.

Qualifications

• Must be a graduate from an accredited school of Nursing.
• Registered Nurse (RN) issued by the state of California.
• BART or BLS at time of hire with commitment to get BART within 6 months of hire date.
• Minimum of two (2) years of current acute care RN experience with liver and/or kidney transplant, or Transplant Coordinator experience.
• Experience coordinating multidisciplinary care across inpatient and outpatient settings, including transfer in and referrals for transplant evaluation.
• Understanding of liver and kidney transplant phases and related guidelines and compliance.

Responsibilities

• Coordinate patient care for the kidney and liver transplant inpatients across all campuses throughout the transplant continuum.
• Collaborate with stakeholders to facilitate outside hospital transfer process upon referral acceptance.
• Provide clinical oversight and escalation of patient changes to surgeons, hepatologists and other transplant stakeholders.
• Serve as leader and advocate for inpatient transplant care and team protocols.
• Navigate patient charts in Epic to access, update, and document information.
• Coordinate with outpatient transplant coordinators to ensure smooth, coordinated patient care.
