Froedtert Health in Milwaukee, WI is seeking a Pre Transplant Coordinator for Liver to provide clinical management and coordination of care for adult pre-transplant patients. You will work with multidisciplinary teams to streamline services and support placement on the national transplant waiting list.

The role requires RN licensure in Wisconsin or multi-state license, with preferred CCTC/CCTN or CCRN certifications; ASN or BSN with relevant transplant experience is expected.

Qualifications

• RN licensure in Wisconsin or NLC multi-state license.
• CCTC/CCTN or CCRN preferred.
• Bachelor’s degree in Nursing preferred.

Responsibilities

• Coordinate clinical care for adult pre-transplant liver patients.
• Collaborate with multidisciplinary teams to facilitate placement on the national waiting list.
• Provide exceptional customer service and program coordination.
