Job summary

Froedtert Health is seeking a Kidney Transplant Coordinator for our US WI Milwaukee facility. This on-site, full-time role supports coordination of care for pre- and post-kidney transplant patients, interfacing with families, staff and the patient care team.

Minimum 3 years of acute care or dialysis experience is required; RN licensure in Wisconsin (or multi-state) is necessary with preferred transplant certifications. Expect 40-hour weeks, M-F days, with shift 1.

Qualifications

• Hands-on patient care in an acute or dialysis setting.
• Strong communication and organizational abilities.
• Ability to work independently and take programmatic initiative.

Responsibilities

• Coordinate patient services across the continuum of care for kidney transplant patients.
• Act as liaison between families, hospital staff and patients pre/post transplant.
• Manage education and clinical coordination with multidisciplinary teams.
