Cleveland Clinic Weston Hospital in Florida is seeking a Liver Transplant Coordinator to oversee transplant patient activities and care planning. You will coordinate with physicians, nurses, and support staff to ensure timely evaluations, test results, and follow-up appointments.

The role requires RN experience, BLS certification, and knowledge of transplant processes. You will work days with on-call requirements and be part of a multidisciplinary team at a top-tier health system.

Qualifications

• BLS Certification through the American Heart Association (AHA) or American Red Cross.
• Two years of RN experience.
• One year of acute care disease-related hospital experience.

Responsabilités

• Oversee all liver transplant patient activities at Weston Hospital.
• Assist in creating care plans and coordinating tests and results.
• Coordinate scheduling of follow-up appointments with the administrative coordinator.
• Review medical information with transplant physicians and initiating appropriate communication.
• Coordinate consultations with other disciplines and obtain follow-up information.
• Coordinate discharge planning for transplant patients and donors.
• Assist in obtaining medications and supplies based on patient insurance coverage.
• Contact referring physicians and arrange follow-up in local area or at Cleveland Clinic.
• Facilitate pre-transplant evaluations including tissue typing and data collection.
• Assemble test results and arrange further testing if indicated.
• Serve as a contact person for pre-transplant evaluation for physicians and insurers.
• Present cases to the Selection Committee.
