Job summary

Qualifications

• BSN required and current Illinois RN license.
• Current CPR/BLS Certification.
• 3–5 years nursing experience in hepatology, gastroenterology, ambulatory care or care coordination preferred.

Responsibilities

• Coordinate comprehensive pre-transplant clinical evaluations and review tests to assess transplant eligibility.
• Serve as liaison among patients, families, hepatologists, transplant surgeons, social workers, dietitians, and financial counselors.
• Prepare and present candidacy data at medical review board meetings.
• Ensure compliance with UNOS, CMS, and Joint Commission standards and maintain national waitlist status.
• Educate patients and families on transplant journey, triage clinical calls, and manage complex medication needs.
• Prepare quality improvement reports, track outcomes, participate in audits, and support program administration.
