Froedtert Health in Milwaukee, WI is seeking a Pre Transplant Coordinator for Liver to provide clinical management and coordination of care for adult pre-transplant patients. You will work with multidisciplinary teams to streamline services and support placement on the national transplant waiting list.

The role requires RN licensure in Wisconsin or multi-state license, with preferred CCTC/CCTN or CCRN certifications; ASN or BSN with relevant transplant experience is expected.

Qualifications

• RN licensure in Wisconsin or NLC multi-state license.
• CCTC/CCTN or CCRN preferred.
• Bachelor’s degree in Nursing preferred.

Job Summary

The Pre Transplant Coordinator- Liver provides clinical management and coordination of care for all adult pre transplant liver patients. The Pre Transplant Coordinator interacts with all necessary disciplines; this ensures the coordination of patient services across the continuum of care to facilitate placement on the national transplant waiting list. The Pre Transplant Coordinator liver must have exemplary customer service skills. Other duties as assigned.

EXPERIENCE DESCRIPTION

Minimum of 3 years of acute care or dialysis experience is required. Experience relevant to Nephrology/Transplant sub-specialties is preferred.

EDUCATION DESCRIPTION

Associate's degree in Nursing with a minimum of 10 years of relevant experience is required or Bachelor's degree in Nursing with a minimum of 3 years of relevant experience is required. Bachelor's degree in Nursing is preferred.

SPECIAL SKILLS DESCRIPTION

Hands on patient experience in an intensive/acute care setting. High degree of organizational skills and excellent communication skills; ability to work independently and take programmatic initiative. Knowledge of data systems, outcomes data and other clinical performance data.

LICENSURE DESCRIPTION

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). Certified Clinical Transplant Coordinator (CCTC) certification, Certified Clinical Transplant Nurse (CCTN), or Acute/Critical Care Nurse (CCRN) is preferred.
