Care Design New York is hiring a Care Manager to help people with intellectual and/or developmental disabilities identify and realize their life goals by developing Life Plans, coordinating supports, and monitoring progress.

The role is hybrid with home visits, office trainings, and occasional travel within New York; a driver's license and reliable transportation may be required outside NYC. Bilingual candidates may receive additional compensation for language skills.

Formación

• Bachelor's degree with 2+ years of relevant experience, OR RN license with 2+ years of relevant experience, OR Master's degree with 1+ year of relevant experience.
• Strong communication and interpersonal skills.
• Excellent organizational and verbal and written communication skills.

Responsabilidades

• Advocate for and with our members to ensure informed decision making, informed consent, and appropriate guardianship.
• Schedule, lead and collaborate with members and interdisciplinary team to develop comprehensive, person-centered Life Plans.
• Implement, update, and monitor Life Plans and conduct Life Plan reviews and approvals.
• Ensure integration of medical, behavioral, social, habilitation, dental, psychosocial, and community-based supports.
• Maintain ongoing contact with the critical people in a member’s life.
• Ensure timely submission of all Life Plan and progress documentation.
• Assist members with maintaining benefits such as Social Security, SSI, Medicaid, Medicare, and Food Stamps.
• Assist members to resolve living problems including housing and utilities and safety.
• Report abuse or neglect immediately when observed or reported.
• A comprehensive job description may be provided during the interview process
