Job summary

Care Design New York is seeking a Care Manager to help individuals with intellectual and/or developmental disabilities identify and pursue their life goals through person-centered Life Plans. The role blends home visits, office trainings, and remote work, with travel if supporting members outside New York City.

Responsibilities include coordinating Life Plans with an interdisciplinary team, advocating for informed decisions, and assisting with benefits and safety.

Qualifications

• Bachelor's degree with 2+ years of relevant experience.
• RN license with 2+ years of relevant experience (case management/service coordination duties included).
• Master's degree with 1+ year of relevant experience.

Responsibilities

• Advocate for members to ensure informed decision making and appropriate guardianship.
• Lead and participate in meetings with members and interdisciplinary team to develop Life Plans.
• Implement, update, and monitor Life Plans and conduct reviews and approvals.
• Coordinate integration of medical, behavioral, social, habilitation, and long-term supports/services.
• Maintain ongoing contact with key people in a member's life.
• Ensure timely submission of documentation (Life Plan, progress notes, etc.).
• Assist members in maintaining benefits (Social Security, SSI, Medicaid, Medicare, Food Stamps) and resolve life challenges.
