Care Design NY in New York is seeking a Care Design New York Care Manager to help people with intellectual and/or developmental disabilities identify and realize their goals by developing, implementing, and monitoring person-centered Life Plans.

You will work in a hybrid environment with home visits, office trainings, and occasional remote work. Strong relational and organizational skills are essential, and fluency in multiple languages may come with additional compensation.

Qualifications

• Strong communication skills, including verbal and written communication, along with strong interpersonal and organizational skills also required.
• Excellent organizational, interpersonal, and verbal and written communication skills required.

Responsabilités

• Advocate for members to ensure informed decision making and guardianship.
• Schedule and lead interdisciplinary meetings to develop Life Plans.
• Implement and monitor Life Plans and conduct reviews.
• Coordinate supports across medical, behavioral, social, habilitation, dental, psychosocial, and community-based services.
• Maintain ongoing contact with the critical people in a member’s life.
• Submit Life Plan and progress notes timely.
• Assist with benefits such as Social Security, SSI, Medicaid and Medicare.
• Help resolve housing, utilities, safety and other living issues.
• Report abuse or neglect immediately.
