Care Design NY is seeking a Care Manager to help individuals with I/DD identify and achieve their Life Plans. The role blends home visits, office trainings, and remote work in a hybrid setup. Fluency in multiple languages and a valid driver’s license may be valued for added compensation.

Formación

• Bachelor’s degree with 2+ years of relevant experience OR RN license with 2+ years OR Master’s degree with 1+ year of relevant experience.
• Strong communication skills, including verbal and written, plus interpersonal and organizational abilities.

Responsabilidades

• Advocate for and with members to ensure informed decision making and appropriate guardianship.
• Schedule and lead meetings and assessments to develop a comprehensive Life Plan.
• Implement and monitor Life Plans and coordinate reviews and approvals.
