Job summary

Advance Care Alliance New York seeks a Care Manager to oversee and coordinate access to services for people with intellectual and developmental disabilities in New York. You will develop, implement, and monitor person-centered Life Plans and advocate for members to meet identified goals and improve quality of life.

The role requires coordinating with family, providers, and an interdisciplinary team, with travel and field work as needed in a flexible work environment.

Qualifications

• Bachelor’s degree with two years of relevant experience, or a nursing license with two years of relevant experience, or a master’s degree with one year of relevant experience.
• Excellent interpersonal, public speaking, and written communication skills.

Responsibilities

• Deliver person-centered care management services in compliance with regulatory standards and agency policies.
• Complete comprehensive assessment/reassessment processes.
• Develop, implement, and monitor Life Plans with an interdisciplinary team.
