Job summary

Care Design New York is hiring a Care Manager to help individuals with intellectual and/or developmental disabilities identify and pursue short- and long-term goals through person-centered Life Plans. The role blends home visits with in-office trainings and occasional travel, with bilingual capabilities and potential additional compensation for fluency in multiple languages.

Eligible applicants include those with a Bachelor's degree plus 2+ years of experience, RN with 2+ years, or a Master's

Qualifications

• Bachelor's degree with 2+ years of relevant experience.
• RN license with 2+ years of relevant experience.
• Master's degree with 1+ year of relevant experience.
• Strong written and verbal communication and organizational skills.

Responsibilities

• Advocate for and with members to support informed decision making and guardianship.
• Schedule and lead interdisciplinary meetings to develop Life Plans.
• Update and monitor Life Plans and conduct reviews for approvals.
• Coordinate medical, behavioral, social, and community supports within Life Plans.
• Maintain regular contact with key people in a member's life.
• Ensure timely submission of Life Plan documents and progress notes.
• Assist members with benefits like Social Security, Medicaid, and food assistance.
• Resolve housing, utilities, safety, and other living issues for members.
• Report abuse or neglect immediately when observed or reported.
