Qualifications

• Bachelor’s degree with 2+ years of relevant experience, OR a Registered Nurse license with 2+ years of relevant experience, OR a Master’s degree with 1+ year of relevant experience.
• Excellent written and verbal communication, plus strong interpersonal and organizational skills.

Responsibilities

• Advocate for members to ensure informed decision making and guardianship.
• Schedule and lead meetings with the interdisciplinary team to develop a Life Plan reflecting needs and goals.
• Implement, update, and monitor Life Plans with reviews and approvals.
• Ensure integration of medical, behavioral, social, habilitation and community supports.
• Maintain ongoing contact with key people in a member’s life.
• Ensure timely submission of Life Plans and progress notes.
• Assist members with maintaining benefits like Social Security, SSI, Medicaid, Medicare, and Food Stamps.
• Help members resolve housing, utilities, judicial system, and safety issues.
• Report abuse or neglect immediately when observed or reported.
