Care Design New York is seeking a Care Manager to help individuals identify and realize their goals by developing, implementing, and monitoring person-centered Life Plans in a hybrid work model.

The role involves home visits, team collaborations, and ongoing documentation, with training provided; drivers license and reliable transportation may be required for out-of-city visits. Generous benefits and PSLF eligibility are offered.

Qualifikationen

• Bachelor’s degree with 2 years relevant experience, OR
• RN license with 2 years relevant experience, OR
• Master’s degree with 1 year relevant experience.
• Strong communication, interpersonal and organizational skills required.
• Bilingual in English and Spanish.

Aufgaben

• Advocate for members to ensure informed decision making and guardianship.
• Schedule and lead meetings with members and interdisciplinary team to develop Life Plans.
• Implement, update and monitor Life Plans and conduct reviews.
• Coordinate supports across medical, behavioral, social, habilitation and community services.
• Maintain ongoing contact with key people in a member’s life.
• Ensure timely submission of documentation (Life Plan, Progress notes, etc.).
• Assist members to maintain benefits like Social Security, Medicaid and Food Stamps.
• Help resolve housing, utilities and safety issues; report abuse when observed.
