Job summary

The role requires coordinating assessments, attending care plan meetings, and supporting residents through diverse recreation and leisure activities.

Qualifications

• Minimum 1 year of experience in a SNF facility.

Responsibilities

• Develops and delivers activity/recreational programming to promote resident engagement.
• Completes the MDS/CAA within five business days of admission and attends Care Plan meetings.
• Performs admissions interviews, documents activities, and writes CAA and care plans.
• Prepares a monthly calendar of activities and posts it in the center.
• Identifies resident activity preferences and health factors affecting participation.
• Completes standardized assessment data and develops individualized care plans.
• Implements strategies to increase involvement in activities despite limitations.
• Plans community activities that involve staff, families and residents.
• Ensures physician approval for programs and maintains precautions lists.
• Offers opportunities for group and individual leisure pursuits and encourages participation.
