Qualifications

• Two years of experience preferred in social/recreational programs in health care.

Responsibilities

• Develops and delivers activity/recreational programming to promote residents' engagement.
• Completes routine assessments and participates in Resident Care Plan Meetings.
• Conducts admissions interviews and documents activity sections of MDS.
• Prepares monthly activity calendars and posts them.
• Identifies resident preferences and health issues affecting activity participation.
• Develops individualized care plans based on resident needs and preferences.
• Promotes group and individual leisure activities with residents, families, and staff.
• Secures physician approval for activity participation and maintains safety lists.
• Responsibilities may change with notice.
