Job summary

Assisted Home Health & Hospice is seeking an LVN in California to provide compassionate, patient-centered home health care. This role involves 12-hour day or night shifts, collaborating with an RN Case Manager and physicians to monitor patient conditions and adjust plans of care as needed.

You will assist the RN/physician with specialized procedures and carry out the established plan of care for a single patient at a time, ensuring safety and quality in every visit.

Qualifications

• Graduate of accredited practical (vocational) nursing program.
• Licensed as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Responsibilities

• Ensure quality and safe delivery of home health nursing services.
• Communicate with RN Case Manager and physician regarding patient condition and needs.
• Assist the RN and/or physician in performing specialized procedures and carrying out the plan of care.

What you will do:

This is shift work. Ensure quality and safe delivery of home health nursing services. Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs. You will assist the RN and / or physician in performing specialized procedures and duties. Assist the RN in carrying out the plan of care.
