Job summary

Assisted Home Health & Hospice in Baldwin Park is seeking a Licensed Vocational Nurse (LVN) for shift care. You will work one-on-one with assigned patients and help ensure safe, high-quality home health nursing services, coordinating with the RN Case Manager and physicians as needed.

The position offers full-time day or night shifts, with opportunities to grow toward RN via tuition reimbursement and a supportive team in a respected agency recognized locally.

Qualifications

• Graduate of an accredited practical (vocational) nursing program.
• Licensed as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Responsibilities

• Work one-on-one with an assigned patients (full-time 40 hour day or night shifts are available)
• Ensure quality and safe delivery of home health nursing services.
• Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs.
• Assist the RN in carrying out the plan of care.
