Job summary

Assisted Home Health & Hospice in Rancho Cucamonga, CA is seeking a Licensed Vocational Nurse (LVN) for shift care. This full-time role offers day or night shifts and the opportunity to work one-on-one with patients in home health settings.

You will collaborate with the RN Case Manager and physician, contribute to ensuring safe, quality care, and support the plan of care for each patient. California LVN license required.

Qualifications

• Graduate of accredited practical (vocational) nursing program.
• Licensed as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Responsibilities

• Work one-on-one with assigned patients (full-time 40 hour day or night shifts are available).
• Ensure quality and safe delivery of home health nursing services.
• Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs.
• Assist the RN in carrying out the plan of care.
