Job summary

Assisted Home Health & Hospice in Ontario, CA is seeking a Licensed Vocational Nurse (LVN) to care for Rosemead patients. The role offers full-time day or night shifts and a supportive team. You will work one-on-one with patients, ensure quality home health nursing, coordinate with the RN Case Manager and physician, and assist the RN with the plan of care.

What you will do:

• Work one‑on‑one with an assigned patients (full‑time 40‑hour day or night shifts are available)
• Ensure quality and safe delivery of home health nursing services.
• Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs.
• Assist the RN in carrying out the plan of care.

Skills , education, and experience:

• Graduate of accredited practical (vocational) nursing program.
• LVN Experienced
• Licensed as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
