Assisted Home Health & Hospice in Canoga Park is seeking LVNs to join our growing team. This shift-based role pays $28/hr with benefits if you average 30 hours weekly; new grads welcome with training provided.

Full-time opportunities across Canoga Park and surrounding areas. You will deliver quality home health nursing to one patient at a time, coordinating with the RN Case Manager and physicians, and assisting with the plan of care.

Qualifications

• Graduate of an accredited practical nursing program.
• Licensed LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
• New grads welcome; training provided.

Responsibilities

• Provide quality home health nursing care one patient at a time.
• Communicate with RN Case Manager and physician on changes in patient condition.
• Assist RN/physician with the plan of care and with specialized procedures.

Pay: $28/hr with benefits offered if you work an average of 30 hrs a week

All shifts available, Day and Night, 12 hours shifts.

What you will do:

This is shift work. Ensure quality and safe delivery of home health nursing services. Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient’s condition and/or needs. You will assist the RN and / or physician in performing specialized procedures and duties. Assist the RN in carrying out the plan of care.

Who you are:

• Graduate of accredited practical (vocational) nursing program. Licensed as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
