Job summary

Sweetwater Care in Butte, MT is seeking an experienced LVN/LPN to provide nursing care within the scope of practice in a long-term care setting. You will follow physician orders and center policies to meet patient needs.

You will document changes in patient conditions, participate in rounds with physicians when necessary, and adhere to narcotic counting and safety procedures. BLS/CPR certification is required or willing to obtain it.

Qualifications

• Active LVN/LPN license in practicing state.
• Long term care experience.
• Proficient in Point Click Care charting.
• BLS/CPR certification or willingness to obtain.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
