Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care within the scope of practice, following physician orders, center policies, and state and local guidelines. The role emphasizes quality patient care in a long-term care setting.

Responsibilities include charting, incident reporting, narcotic controls, and monitoring patient changes. The ideal candidate has long-term care experience, PC charting familiarity, and CPR/BLS certification or willingness to obtain.

Qualifications

• Requires experience in long-term care settings.
• Proficient with PointClickCare charting software or similar systems.
• Current LVN/LPN license in practicing state; BLS/CPR certification required or willing to obtain.

Responsibilities

• Follow established charting procedures and document incidents accurately.
• Adhere to narcotic counting, pharmacy orders, and restraint reduction processes.
• Document changes in patient conditions (nutrition, infections, skin issues, weight changes, etc.).
• Utilize the Quality Life Manual for patient placement and monitoring in programs.
• Notify relevant departments of room changes, LOA, new admissions, or discharges.
• Huddle with physicians during rounds when needed.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
