Job summary

Sweetwater Care in Butte, MT is seeking a licensed practical nurse (LPN/LVN) to provide nursing care within scope of practice, following physician orders and center policies. The role requires long term care experience, PCC charting, current LPN/LVN license in practicing state, and BLS/CPR certification; nursing management experience is a plus.

You will participate in rounds with physicians as needed and ensure documentation of patient changes, adherence to procedures, and quality of life

Qualifications

• Requires active LPN/LVN license in practicing state.
• Experience with long-term care settings and patient monitoring.
• Proficiency with PointClickCare charting software.
• Current BLS/CPR certification or willingness to obtain.
• Nursing management experience is a plus but not required.

Responsibilities

• Follows established procedures for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents patient condition changes (nutrition, infections, skin issues, weight, etc.).
• Uses Quality Life Manual to identify, place, and monitor patients in programs.
• Document and notify departments of room changes, LOA, admits, or discharges.
• Makes rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
