Job summary

Sweetwater Care in Dillon, MT seeks an attentive Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) to provide nursing care within scope of practice, following physician orders and center policies.

You will document patient conditions, assist with charting, participate in rounds with physicians, and help manage patient care plans under state guidelines. This full-time role offers a stable environment at a community-focused facility in Montana.

Qualifications

• Experience in long-term care settings.
• Familiarity with PointClickCare charting.
• Licensed LVN/LPN in the state.
• BLS/CPR certification or willingness to obtain.
• Nursing management experience preferred but not required.

Responsibilities

• Follow established procedures for charting and incident reporting.
• Adhere to narcotic counting, pharmacy orders, and restraint reduction procedures.
• Document changes in patient conditions, including nutrition, infections, skin issues, or weight loss.
• Use the Quality Life Manual to identify, place, and monitor patients in appropriate programs.
• Document and notify relevant departments about room changes, LOA, new admits, or discharges.
• Make rounds with physicians when necessary.

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
