Job summary

Health Connect America seeks a Licensed Therapist to deliver evidence-based mental health services in Virginia. You will conduct assessments, develop treatment plans, and provide individual/family/group therapy to diverse clients in a community-based setting.

The role requires state licensure (LPC/LMFT/LCSW) and a Master’s in a related field, with strong communication and documentation skills. You may engage in outreach and care coordination to support clients and families.

Qualifications

• Master's Degree in Social Work, Counseling, or a related field is required.
• Current Virginia licensure as an LPC, LMFT, or LCSW is required.
• Experience conducting assessments, treatment planning, crisis intervention, and evidence-based therapy services is preferred.
• Trauma-informed care knowledge and best practices in mental health services are expected.
• Excellent communication, documentation, collaboration, and problem-solving skills are essential.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and inform individualized treatment planning.
• Develop treatment plans with measurable goals in collaboration with clients and families.
• Provide individual, family, and group therapy using evidence-based approaches.
• Monitor client progress and coordinate referrals to additional resources as needed.
• Provide crisis intervention and safety planning to ensure client safety.
• Maintain timely clinical documentation and comply with regulatory requirements.
