Job summary

Health Connect America is seeking a Licensed Therapist to provide high-quality mental health services to clients across our service areas. This role involves conducting assessments, developing treatment plans, coordinating with care teams, and delivering evidence-informed therapy in collaboration with clients and families.

Applicants must hold a Master’s Degree in Social Work, Counseling, or a related field and be eligible for or hold one of Ohio’s licenses (LSW, LISW, LPC, LMFT, LPCC).

Qualifications

• Master’s degree is required.
• Active Ohio licensure is required or eligible under LSW/LISW/LPC/LMFT/LPCC.
• Strong documentation and collaboration skills.

Responsibilities

• Conduct biopsychosocial assessments to determine client needs and interventions.
• Develop and implement individualized treatment plans with clients and families.
• Deliver individual, group, and family therapy.
• Maintain accurate, timely documentation and billing information.
• Coordinate with multidisciplinary teams and community resources.
• Engage in outreach to promote services and referrals.
• Respond to crises with appropriate safety planning and interventions.
