Job summary

Health Connect America in Virginia is seeking a Licensed Therapist to deliver high-quality, evidence-based mental health services across clients in homes, communities, and offices. You will conduct assessments, develop treatment plans, and provide individual, family, and group therapy with a focus on trauma-informed care and collaborative care within multidisciplinary teams.

A current Virginia LPC/LMFT/LCSW license is required, as is a master’s degree in a related field.

Qualifications

• Master's Degree in Social Work, Counseling, or related field.
• Current Virginia licensure as an LPC, LMFT, or LCSW.
• Experience with assessments, treatment planning, crisis intervention, and evidence-based therapy.
• Ability to work effectively within multidisciplinary teams.

Responsibilities

• Conduct biopsychosocial assessments and utilize standardized tools to identify client needs, establish diagnoses when appropriate, and inform treatment planning.
• Develop treatment plans with measurable goals and objectives in collaboration with individuals, families, and treatment team members.
• Provide individual, family, and group therapy using evidence-based interventions addressing anxiety, depression, trauma, and related concerns.
• Monitor client progress, maintain contact and follow-up, adjust plans as needed, and coordinate referrals to additional resources.
• Provide crisis intervention, safety planning, and coordination with emergency resources to ensure client safety and continuity of care.
• Maintain timely, accurate clinical documentation while ensuring compliance with payer, regulatory, licensing, accreditation, and organizational requirements.
