Job summary

Health Connect America seeks a licensed therapist to deliver high-quality behavioral health services to clients across our communities. You will conduct assessments, develop treatment plans, and provide individual, family, and group therapy using evidence-based approaches.

As part of a multidisciplinary team, you will monitor progress, coordinate referrals, and participate in care coordination and outreach activities to support whole-person wellness.

Qualifications

• Master's Degree in Social Work, Counseling, or a related field.
• Current Virginia licensure as an LPC, LMFT, or LCSW.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and inform individualized treatment planning.
• Develop treatment plans with measurable goals in collaboration with individuals and families.
• Provide individual, family, and group therapy using evidence-based interventions.
• Monitor client progress, maintain contact and follow-up, coordinate referrals as needed.
• Provide crisis intervention, safety planning, and coordination with emergency resources.
• Maintain timely, accurate clinical documentation and ensure compliance with requirements.
