Fairfield Community Health Center in Lancaster, Ohio seeks a Licensed Social Worker or Counselor to deliver direct behavioral health services in multiple settings, including office, school, community, or telehealth. The role supports trauma-informed, strengths-based care and requires active supervision and adherence to professional ethics.

Active LSW or LPC in Ohio is required; Bachelor’s degree in Social Work is mandatory, and a Master’s is preferred.

Qualifications

• Active Ohio LSW or LPC license required.
• Bachelor's degree in Social Work or related field required.
• Master's degree in Social Work or Counseling preferred.
• Experience with multi-need individuals and families.
• Ability to bill Medicaid without restrictions.

Responsabilités

• Conduct biopsychosocial assessments and formulate provisional diagnoses under supervision.
• Provide therapeutic interventions within licensure scope and supervision.
• Develop and update treatment plans using trauma-informed, strengths-based principles.
• Coordinate with internal teams to ensure continuity of care.
• Maintain Medicaid-compliant documentation.
• Participate in clinical supervision, case reviews, and trainings.
• Meet productivity and quality expectations.
• Provide services in office, school, community, or telehealth settings; travel may be required.
