Job summary

Fairfield Community Health Center in Lancaster, Ohio seeks a Licensed Social Worker or Counselor to provide behavioral health services across office, school, community, or telehealth settings. Active LSW or LPC license in Ohio is required; travel within the service area may be needed.

Salary ranges from $60,000 to $80,000 annually, based on experience and licensure. Responsibilities include biopsychosocial assessments, developing trauma-informed treatment plans, and Medicaid-compliant

Qualifications

• Bachelor’s degree in Social Work or related field required.
• Master’s degree in Social Work or Counseling preferred.
• Licensed and active in Ohio as LSW or LPC required.
• Experience with multi-need individuals and families.
• Excellent communication skills, both oral and written.
• Excellent organizational skills.
• Trauma-informed engagement skills.
• Knowledge of community resources and behavioral health systems preferred.
• Ability to bill Medicaid without restrictions.

Responsibilities

• Conduct biopsychosocial assessments and provisional diagnoses under supervision.
• Provide therapeutic and supportive interventions within licensure and supervision.
• Develop and update treatment plans using trauma-informed, strengths-based principles.
• Coordinate with internal teams to ensure continuity of care.
• Maintain Medicaid-compliant documentation.
• Participate in supervision, case reviews, and trainings.
• Provide services in office, school, community, or telehealth settings; travel may be required.
