Job summary

Fairfield Community Health Center in Lancaster, Ohio is seeking a Licensed Social Worker or Counselor to deliver direct behavioral health services to individuals and families with diverse needs. Active LSW or LPC license in Ohio is required. Flexible hours and travel within the service area may be required, with a strong emphasis on Medicaid

Qualifications

• Active Ohio LSW or LPC license required
• Experience with multi-need individuals and families
• Experience with Medicaid billing is preferred
• Ability to travel as needed for program assignments
• Strong written and verbal communication skills
• Excellent organizational skills
• Trauma-informed engagement skills

Responsibilities

• Conduct biopsychosocial assessments and formulate provisional diagnoses under supervision
• Provide therapeutic and supportive interventions within scope of licensure and supervision
• Develop and update treatment plans using trauma-informed, strengths-based principles
• Coordinate with internal teams to ensure continuity of care
• Maintain timely, Medicaid-compliant documentation
• Participate in clinical supervision, case reviews, interdisciplinary meetings and training
• Meet productivity and quality expectations
• Provide services in office, school, community, or telehealth settings; travel may be required
