Job summary

Fairfield Community Health Center, Lancaster, Ohio, is seeking a Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) to deliver behavioral health services across office, school, community, and telehealth settings.

The position requires active Ohio licensure, strong communication skills, and experience with trauma-informed care. Responsibilities include biopsychosocial assessments, treatment planning, and Medicaid documentation, with collaboration across care teams.

Qualifications

• Bachelor’s degree in Social Work or related field required
• Master’s degree in Social Work or Counseling preferred
• Licensed and active in Ohio as LSW or LPC required
• Experience with multi-need individuals and families
• Ability to bill Medicaid without restrictions

Responsibilities

• Conduct biopsychosocial assessments and provisional diagnoses under supervision
• Provide therapeutic and supportive interventions within licensure and supervision
• Develop and update treatment plans using trauma-informed, strengths-based principles
• Coordinate with internal teams to ensure continuity of care
• Maintain Medicaid-compliant documentation
• Participate in supervision, case reviews, and trainings
• Meet productivity and quality expectations
• Provide services in office, school, community, or telehealth settings; travel may be required
