Qualifications

• Masters degree in social work (MSW) required.
• Credentials from multiple states; primary state credentials required at hire, others post-hire.
• Independent Clinical Social Worker License (DC) within 6 months of hire; VA license within 6 months; MD license within 6 months.

Responsibilities

• Assess members health status, functional limitations, psychological status, social support systems, resources, environmental factors, and response to treatment.
• Use motivational interviewing to identify readiness for change and create care plans.
• Provide supportive counseling and education to members, families, and caregivers, including end-of-life issues and Advance Directives.
• Document case management activities consistently per organizational standards.
• Facilitate access to local, state, and federal programs (e.g., Medicaid, Medicare) and community resources.
• Coordinate care across behavioral health and substance abuse services.
• Manage caseload and guide members through health-related legal processes and care transitions.
