Job summary

Middletown Health and Rehab is seeking a Social Services professional to assist the Director in meeting the emotional and social needs of residents in a skilled nursing setting. You will help plan and direct the Social Services Department in alignment with federal, state, and local guidelines.

Applicants should have a high school diploma, a current Social Work license, and experience in long-term care and behavioral health. English proficiency and the ability to follow instructions are essential.

Qualifications

• Must possess a high school diploma or equivalent.
• Current, unencumbered Social Work license as required by state/federal regulations.
• Experience in Long Term Care and behavioral health.
• Experience in healthcare settings including social services or direct care is preferred.
• Must communicate in English and follow verbal and written instructions.

Responsibilities

• Help plan, organize, implement, evaluate and direct the Social Services Department operations.

Position Summary:

The primary purpose of your job is to assist the Director to ensure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. You willassist in planning, organizing, implementing, evaluating, and directing the overall operation of the community’s Social Services Department inaccordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures.Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community's administration and Social Services Policies and Procedures.

Education and Qualifications:

• Must possess a high school diploma or equivalent.
• Possession of a current, unencumbered license to practice as a Social Work within the state in which you work, if required by state/federal regulations.
• Must have experience in Long Term Care as well as behavioral health
• Experience working in healthcare, either social services, direct care or hospitality, is preferred.
• Must be able to communicate in English and have the ability to follow verbal and written instructions.

Shift & Wage:

• Full-Time Opportunity Available
• Shifts Needed: Monday - Friday, Days
  • Occasional Weekends as Needed
• Wage: $30.00 per hour, Based on Experience
