Job summary

Matthews Health & Rehab Center is seeking a Social Services professional to support the Director in meeting residents' emotional and social needs within a skilled nursing setting. You will assist in coordinating the Social Services Department in line with federal, state, and local standards and the facility's policies.

The role requires LTAC experience, a valid Social Work license where applicable, and the ability to communicate in English while following verbal and written instructions.

Qualifications

• High school diploma or equivalent.
• Current, unencumbered Social Work license in the state where you work, if required by regulations.
• Experience in Long Term Care and behavioral health.
• Healthcare/social services or hospitality experience preferred.
• Ability to communicate in English and follow verbal and written instructions.

Responsibilities

• Support the Director in ensuring residents' medically related emotional and social needs are met on an individual basis.
• Assist in planning, organizing, implementing, evaluating and directing the Social Services Department operations.
• Adhere to health, sanitary and infection control policies and the community's policies and procedures.

Education

Position Summary:

The primary purpose of your job is to assist the Director to ensure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. You willassist in planning, organizing, implementing, evaluating, and directing the overall operation of the community’s Social Services Department inaccordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures.Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community's administration and Social Services Policies and Procedures.

Education and Qualifications:

• Must possess a high school diploma or equivalent.

• Possession of a current, unencumbered license to practice as a Social Work within the state in which you work, if required by state/federal regulations.

• Must have experience in Long Term Care as well as behavioral health

• Experience working in healthcare, either social services, direct care or hospitality, is preferred.

• Must be able to communicate in English and have the ability to follow verbal and written instructions.
