Job summary

The Saber Health Group is seeking a dedicated professional to support the Director in ensuring residents' emotional and social needs are met in a compliant skilled nursing facility setting. You will help plan, organize, implement, and evaluate the Social Services Department's operations, following federal, state, and local standards and our policies to deliver compassionate, person-centered care.

Excellent communication with residents, families, and care teams is essential as you uphold

Qualifications

• Must possess a high school diploma or equivalent.
• Possession of a current, unencumbered license to practice as a Social Work within the state in which you work, if required by state/federal regulations.
• Must have experience in Long Term Care as well as behavioral health.
• Experience working in healthcare, either social services, direct care or hospitality, is preferred.
• Must be able to communicate in English and have the ability to follow verbal and written instructions.

Responsibilities

• Assist the Director to ensure residents' medically related emotional and social needs are met on an individual basis.
• Assist in planning, organizing, implementing, evaluating, and directing the Social Services Department operations in accordance with federal, state, local standards and policies.
• Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community's administration and Social Services Policies and Procedures.

Position Summary:

Education and Qualifications:
