Job summary

Telos Health Systems, Inc. in Norfolk, VA is seeking a Psychotherapist (LCSW, LPC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to deliver mental health evaluations and psychotherapy in Skilled Nursing Facilities and Assisted Living Facilities.

The position offers both part-time (2–3 days/week) and full-time schedules with benefits, administrative support, and opportunities to grow across care environments. Licensure in applicable state is required.

Qualifications

• Licensed mental health professional with state licensure in practice area.
• Ability to work independently within a clinical team and maintain documentation.
• Experience in geriatric or long-term care settings is a plus.

Responsibilities

• Conduct mental health evaluations and diagnostic assessments.
• Provide individual psychotherapy and ongoing treatment.
• Develop and update individualized treatment plans with a multidisciplinary team.
• Maintain compliant clinical documentation.
• Coordinate care with facility staff and healthcare professionals.
• Adhere to federal, state, and organizational standards.
