Job summary

Telos Health Systems seeks a Psychotherapist (LCSW, LCMHC, LMFT, or Psychologist — PsyD/Ph.D.) to deliver mental health evaluations and psychotherapy in Skilled Nursing and Assisted Living facilities. Flexible schedule with part-time or full-time options and strong administrative support.

The role offers autonomy within a clinical team, travel between Sparta, NC and other NC locations, a sign-on bonus up to $7,500, and benefits for full-time staff.

Qualifications

• Licensed clinician in the state with LCSW, LCMHC, LMFT, or PsyD/Ph.D.
• Active, unrestricted license in the state of practice
• Master’s or Doctoral degree in a mental health field
• Ability to work independently yet collaborate within a clinical team

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations
• Provide individual psychotherapy and ongoing treatment services
• Develop and update individualized treatment plans with a multidisciplinary team
• Maintain accurate, timely, and compliant clinical documentation
• Coordinate care with facility staff and healthcare professionals
• Adhere to federal, state, and organizational standards
