Job summary
CareDiem Home Health in Illinois seeks compassionate and reliable LPNs to provide skilled home health nursing to patients in Poplar Grove and surrounding areas.

Qualifications
- Current LPN license in Illinois.
- 3+ years nursing experience, home health preferred.
- CPR certification (BLS) required.

CareDiem Home Health is seeking compassionate and reliable LPNs to provide skilled home health nursing services to patients in POPLAR GROVE AND SURROUNDING AREAS. Our mission is to help clients remain safely at home while receiving top-quality, personalized care.
Deliver skilled nursing care under a physician's order
Complete assessments, medication administration, and wound care
Monitor and document patient progress using our electronic charting system (Axxess)
Educate patients and families on health conditions and care
Communicate with physicians and the care team
Travel to patient homes
Current LPN license in the state of Illinois
3+ year nursing experience (home health preferred)
CPR certification (BLS)
Reliable transportation, valid driver's license, and auto insurance
Strong clinical judgment, communication, and independence
Flexible scheduling (FT, PT, PRN, Per Visit Based)
