Job summary

CareDiem Home Health is seeking compassionate LPNs to deliver skilled nursing care in patients’ homes across LaSalle County. You’ll complete assessments, manage medications, and document progress using Axxess, while educating families and coordinating with physicians.

Ideal candidates hold an Illinois LPN license, have 3+ years of nursing experience (home health preferred), CPR/BLS certification, and reliable transportation.

Qualifications

• Current LPN license in Illinois
• 3+ years nursing experience (home health preferred)
• CPR certification (BLS)
• Reliable transportation, valid driver’s license, and auto insurance
• Strong clinical judgment, communication, and independence
