Job summary

CareDiem Home Health is seeking compassionate LPNs to provide skilled nursing care to patients in Bureau County and surrounding areas. You will deliver care under physician orders, perform assessments, med administration, and wound care, and document progress in Axxess.

This role requires a current Illinois LPN license, CPR/BLS certification, and at least 3+ years of nursing experience, preferably in home health. Travel to patient homes is required; flexible scheduling options are available.

Qualifications

• Current LPN license in Illinois
• 3+ year nursing experience (home health preferred)
• CPR certification (BLS)
• Reliable transportation and valid driver’s license
• Strong clinical judgment and communication
